Greenwood Village, CO – OSHA has cited roofing company Aspen Contracting Inc. – based in Lee’s Summit, Missouri – and subcontractor J Cuellar LLC – based in West Bend, Wisconsin – for exposing employees to dangerous fall hazards at a Fountain, Colorado construction site. The companies face proposed penalties totaling $147, 998.
During a July 2018 inspection, OSHA observed employees of J Cuellar LLC re-roofing a home without fall protection, and cited it for a fall protection violation. OSHA cited Aspen Contracting Inc. for repeatedly failing to protect workers from fall hazards, and for failing to conduct frequent and regular inspections of its job site.
Wichita, KS – Midwest Drywall Company Inc. is facing stiff penalties for safety violations after part of a forklift fatally crushed an employee at the company’s Wichita, Kansas, facility.
The inspection revealed that a hydraulic boom hoist cylinder came free from the supporting slings, and landed on top of the employee. OSHA cited the company for serious violations of forklift, machine guarding, and control of hazardous energy standards.
Proposed penalties total $77,604.
Clearwater, NE – Thiele Dairy has been cited penalties totaling $78,899 for failing to develop and implement safety and health programs related to grain bin entry after an employee suffered fatal injuries.
OSHA inspectors determined that an operating sweep auger lacerated an employee’s leg as he attempted to remove corn from inside a grain bin at the Clearwater, Nebraska, dairy.
OSHA cited the company for eight serious violations of the grain bin safety standard, and one other-than-serious violation for failing to report the fatality within the required eight hours.
