OSHA has cited an Ohio-based glass manufacturer $724,380 for multiple serious violations, exposing workers to safety and health hazards.

An inspection initiated under OSHA’s Site-Specific Targeting program found that Fuyao Glass America Inc. had repeatedly failed to implement and enforce safety and health programs at its Moraine, OH plant.

OSHA found nine repeated and 13 serious violations at Fuyao, including ones for

  • Electrical safety;
  • Failing to evaluate permit-required confined spaces;
  • Failure to train workers on lockout/tagout and entering confined spaces;
  • Failure to install machine guarding; and
  • Lack of hearing protection, fall protection or personal protective equipment.

The company’s plant has been cited 12 times in the past four years for similar violations.
