Trench Collapse Leaves One Worker Dead

MENTOR, OH – OSHA Investigators are trying to figure out what caused a deadly trench collapse during construction work on a water line.

City officials said the collapse happened at Heisley Road near Jackson Street, where crews were relocating the water line.

OSHA said the 28-year-old worker suffered serious injuries when he became trapped in a trench between 6 and 8 feet deep, and was declared dead at the hospital.

OSHA cited Aqua Ohio twice in 2013 for serious safety violations. The company was fined $5,400 after it failed to provide ladders for employees working in trenches.
